Advance Care Planning in Queensland

In Queensland the components of an advance care plan are:

  • Enduring Power of Attorney – to appoint a substitute decision maker
  • Advance Health Directive – a legal document where you can record your wishes regarding future medical treatments. There is an approved form that can be used, although it may be in another form.
  • Statement of Choices – to record your wishes regarding future medical treatments, to inform your substitute decision maker and doctor to assist them in making decisions for you.

 

Forms:

Enduring Power of Attorney short form – for appointing the same attorney(s) for both financial and personal matters, or for appointing an attorney for only financial matters or only personal matters

Enduring Power of Attorney long form – for appointing different attorneys for personal matters and financial matters

Enduring Power of Attorney revocation form – for revoking (cancelling) the appointment of a person or persons as your enduring power of attorney

Advance Health Directive – for recording your wishes and/or directions regarding your future health care for various medical conditions

Statement of Views – given on behalf of people who do not have capacity to make health care decisions
