We can guide you through advance care planning and help with:
- the advance care planning process
- documents you need to record your choices in your state or territory, and how to store them
- starting the conversation about advance care planning with family, loved ones, health and aged care providers to make sure your goals, values and preferences are heard
- choosing or being a substitute decision-maker
- understanding the role and responsibilities of health professionals.

You can use an interpreter to speak to us in a language other than English.
The National Advance Care Planning Advisory Service takes calls through an interpreter service.
Use an interpreter
- Call 13 14 50, Monday to Friday, 9 am to 5 pm (AEST).
- Say the language you need.
- Stay on the phone for your interpreter. You might wait for a few minutes.
- Ask the interpreter to call Advance Care Planning Australia on 1300 208 582.
- Talk with our team with help from the interpreter.
Calls are charged at the cost of a local call (except calls from mobiles).
